Lingual braces are fixed braces bonded to the tongue side of the teeth. A bracket is fitted to each tooth, and a wire runs through the brackets and moves the teeth as it is changed and adjusted. The brackets are often made for each tooth in a laboratory from a scan or impressions, so that they fit the shape of the inside surface, which is less even than the front.2
The great advantage is that they are hidden from the front. The drawbacks are real. Studies have found that speech difficulty and tongue soreness are more likely than with front braces, and that oral hygiene problems are more common. They are harder to see and reach, adjustment visits can take longer, and the clinician needs special training to place and adjust them.1
