All-on-4 is a name for a method of supporting a fixed bridge of about ten to twelve teeth on four implants in one jaw. The implants are placed in the front of the jaw, and the two at the back are tilted by an angle, so that they can be longer and can sit in the denser bone at the front of the sinus or nerve, instead of needing bone to be built up first. The bridge is attached to the four implants.4
A fixed bridge is made after the implants have bonded with the bone. In many cases a temporary bridge is attached soon after surgery, so that you leave with teeth. A review of studies of immediately loaded upper jaw bridges found success rates that were comparable with later loading, with the benefit of a shorter treatment time. A temporary bridge is not always possible, and the plan depends on how firmly the implants sit.2

If only a few teeth are missing, see multiple teeth implants or a bridge instead. Teeth that cannot be saved are removed by tooth extraction, sometimes at the same visit as the implants. Gum disease needs periodontal treatment first.

It depends on whether one or both jaws are treated, the implant system, the scans and surgical guide, any tooth removal and bone work, the material and design of the temporary and final bridges, the number of visits and the follow-up. All-on-4 is a way of replacing all the teeth in one jaw with a fixed bridge held by four implants. The two implants at the front are placed upright, and the two at the back are tilted so that they use the bone that is there and can be longer, which can avoid a bone graft in some patients.
In selected cases, a fixed temporary bridge is attached soon after the implants are placed. It depends on how firmly the implants sit, the quality of the bone and your bite. A temporary bridge is for use while the implants heal, and we do not promise it before the scans and the findings at surgery.
Planning takes one or more visits, surgery usually takes one visit for the jaw, and the implants then need months to bond with the bone. A final bridge is made after healing is confirmed. The total commonly takes several months, and we give an estimate after your scans.
It is a different kind of treatment. A fixed implant bridge does not come out and does not rest on the gums, but it needs surgery, costs more time and care, and has risks. A denture needs no surgery but can move and may need relining. We compare them for your mouth.
No. All-on-6 uses six implants per jaw. Reviews of studies on upper jaw bridges found no clear difference in how many implants and bridges survived with four or six implants, but there was more bone loss around the implants with four. Dr. Shah explains which suits your bone and bite.
Clean it every day with a soft brush, a water flosser and interdental brushes or floss threaders under the bridge, as we show you. Regular professional cleaning and check-ups are needed so that the gum and bone around each implant are checked. Plaque around implants can cause gum and bone inflammation.
People with untreated gum disease or infection, heavy smoking, uncontrolled diabetes, certain medicines or radiotherapy to the jaw, or strong grinding that has not been managed need caution or another plan. Some people with very little bone need a different approach. We tell you honestly if All-on-4 is not right.
