The choice between four and six implants depends on the bone, the bite and the design of the bridge. A systematic review of upper jaw bridges found no clear difference in implant and bridge survival, or in mechanical and biological complications, between four and six implants. It did find more bone loss around the implants in the four-implant group, so six may be preferred when the bone allows.1

Gum disease and infection are treated, teeth that cannot be saved are removed, and any bone grafting is done and given time to heal.4
Under local anaesthesia, six implants are placed along the jaw in the planned positions, often using a surgical guide made from the scan.
In selected cases, a temporary fixed bridge is attached soon after surgery. If the implants are not firm enough, a temporary denture is used.
The implants bond with the bone over several months. You eat soft food, and review visits check the gums, the implants and the temporary teeth.
After healing is confirmed, impressions or scans are taken, and the final bridge is made, tried in, adjusted and fixed. You are shown how to clean it.

All-on-6 replaces all the teeth in one jaw with a fixed bridge held by six implants. The extra implants share the chewing load across a longer span, and they can give support if one implant has a problem. It needs more bone than a four-implant plan, or some bone building.
Six implants spread the load over more points and can suit people with a strong bite, a long arch or softer bone. If one implant is lost, the bridge may still be supported by the others. A review of studies of upper jaw bridges found no clear difference in implant and bridge survival between four and six implants, but less bone loss around the implants with six.
Sometimes. Six implants need enough bone in more places. If the bone at the back of the upper jaw is thin, a graft or sinus procedure may be needed before or during surgery. The scan shows whether this applies, and Dr. Shah explains the options.
The bridge is fixed to the implants and is not taken out at home. It can be unscrewed by the dentist for cleaning and repair. It is designed with a shape that lets you clean beneath it.
Planning takes one or more visits and surgery usually takes one visit per jaw. The implants then need months to bond with the bone before the final bridge. A temporary bridge may be possible in selected cases. We give an estimate after your scans.
Clean it every day with a soft brush, a water flosser and interdental brushes or threaders under it. Attend regular professional cleaning, because plaque around implants can inflame the gum and bone. Tell us if a screw feels loose or a tooth chips.
People with untreated gum disease or infection, heavy smoking, uncontrolled diabetes, bone-affecting medicines or radiotherapy to the jaw need caution. Very thin bone may need a different plan, and heavy grinding needs managing. We tell you honestly if All-on-6 is not right.
